What to Bring

To make the most of your appointment, please arrive a few minutes early and bring the following items with you:

  • Your health and/or dental insurance information
  • A list of current and past medical conditions (e.g., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.)
  • A list of current medications
  • Referral forms and/or any x-rays, if applicable
  • A valid driver’s license or photo ID
  • A form of payment

Parents of Minors
Patients under the age of 18 must be accompanied by a parent or legal guardian for their consultation.

Your first visit with our board-certified surgeons is an opportunity for us to get to know each other and outline a treatment plan tailored to your specific condition and goals for improved oral health. During this visit, we will:

  • Review your medical history
  • Conduct a comprehensive oral examination
  • Utilize advanced 3D imaging
  • Discuss your anesthesia options

This is also a great time to ask any questions you may have about the procedure or your treatment plan.
